ASSISTANCE POLICY
The following policy applies to those who submit the requested documentation, do not qualify for Handicapped Children’s Service and/or do not have insurance coverage for the requested service.
Maximums are per client, per calendar year.  All financial assistance must be requested prior to purchase, repair, rental or any rendering of service, to assure that all client/consumers receive the best quality of services from vendors.

MOBILITY (Wheelchairs, walkers, braces, crutches, etc.)
$2,000 MAXIMUM
Uninsured Clients -80%
Insured Clients - 50% of balance after deductible
COMMUNICATION DEVICES (Incl. hearing aides)
$500 MAXIMUM
Uninsured Clients - 80%
Insured Clients - 50% of balance after deductible
EQUIPMENT RENTAL (3 mo. max., not to exceed purchase price)
EQUIPMENT REPAIRS
(A determination of abuse or neglect warrants disqualification)
$600 MAXIMUM
Uninsured Clients - 80%
Insured Clients - 50% of balance after deductible
SHOES(prescription only, 3 pair/yr./maximum
Uninsured Clients - 80%
Insured Clients - 50% of balance after deductible
PHYSICAL, SPEECH & OCCUPATIONAL THERAPY (School age clients must provide letter of denial from Special Education school system)
$2,000 MAXIMUM
Uninsured Clients - 80%
Insured Clients - 50% of balance after deductible
EVALUATIONS
$300 MAXIMUM for speech/physical, $80 for physiological.
Uninsured Clients - 80%
Insured Clients - 50% of balance after deductible
